How this service is coordinated and covered
The hospice team coordinates the plan of care with the resident, family, attending physician, and assisted living staff. This coordination does not transfer the community’s usual responsibilities to hospice.
Room and board and other facility charges may be separate from the hospice benefit. Coverage depends on the patient’s circumstances and should be confirmed during referral review. Review Medicare hospice coverage.
