For senior living and facility care teams

When a resident may benefit from hospice, Caritas Hospice helps senior living and facility care teams prepare a referral, coordinate responsibilities, and keep the resident and family informed.

Facility Referrals

How facility teams can prepare a hospice referral

When to call

  • A resident has a life-limiting illness and comfort-focused care may be appropriate.
  • Symptoms, functional decline, or care needs are becoming harder to manage.
  • The resident, family, and facility team need guidance before a change in the care plan.

Information to prepare

  • The resident’s current diagnosis, physician contact, and a summary of recent changes.
  • Available clinical notes, medication information, and the family or decision-maker’s contact details.
  • The resident’s current care setting and the best facility contact for coordination.

Coordinate responsibilities

  • Facility responsibilities include its services, staffing, policies, and resident support.
  • Hospice responsibilities include the hospice plan of care and services coordinated with the resident, family, physician, and facility team.
  • Both teams clarify contacts, changes to report, and how the care plan will be communicated.

Use secure referral steps

  • Call 214-506-8422 or begin with the patient referral page.
  • Confirm the appropriate secure transfer process with the care team before sending referral documentation.
  • Do not send patient information or medical records through ordinary email.
